FHLB offers a Consumer Driven Health Plan ("CDHP") through Anthem. Employees and dependents enrolled in the medical plan are provided vision insurance, through EyeMed, at no cost. The CDHP offers a broad network of healthcare professionals and facilities. Coverage is offered the first of the month following employment. Dependent children may stay enrolled in the medical and vision plans to age 26.

Marathon Health
Employees and dependents enrolled in FHLB’s CDHP may visit any of Marathon Health’s locations for acute, preventative or behavioral health care. Preventative visits are covered 100%. Other services provided with a small charge.

Dental Coverage
FHLB offers dental insurance through Delta Dental. Preventative visits are covered at 100% within the network. Dependent children may stay enrolled in the dental plan to age 26.

Health Savings Account
High deductible plans allow employees to open a Health Savings Account ("HSA"). FHLB provides a contribution to an employee’s HSA based on the plan and level of coverage elected when the account is with the FHLB sponsored HSA partner. Deductibles, co-insurance, prescription co-payments and other out of pocket expenses related to medical, dental or vision treatment may be paid with HSA dollars.
